PAR-Q FORM (Physical Activity Readiness Questionnaire)
Please send back your completed PAR-Q form in good time before your first class.
This is to ensure I am aware of any conditions, injuries or ailments you may have as well as
your physical fitness level. This will help me design better-suited sessions.
Usually you only need to fill in the PAR-Q form once. However, in case something changed
(new conditions or injuries), please complete a new PAR-Q form.
BOOKING AND PAYMENT
Your first group class is only £5.00 to try. Subsequent classes are as per the price list.
Please make sure you pay for your classes in advance to secure your space. This also applies to Pay as You Go option.
Where not possible, please bring exact amount owed to the class.
Please do however note that if the class has a waiting list,
you might not be granted your space unless payment has been settled prior to the class.
Payments can be made: in cash, by cheque, or by bank transfer.
CANCELLATIONS AND NON-ATTENDANCE
Where possible, please give at least 24 hours' notice if you need to cancel your class
so I can offer your space to another participant.
Subject to availability, you can attend an alternative class as substitute to the cancelled session. Where possible,
attend it within a month from the missed session to avoid build-up of missed classes.
In case your regular teacher is not available, the class may be covered by another instructor
or, where not possible, cancelled.
There will be 46 working weeks in a calendar year, allowing the teacher to be away for 6 weeks
(holidays, sickness, emergencies, etc.) without having to arrange a cover.
Please note that refunds are not possible.
When you purchase 5 classes in a bundle, ideally you will attend them at regular intervals (at least one a week)
for best results. However, it is not always possible, and flexibility is offered.
The 5 classes, however, must be used up within 2 months.
